Celebrating Social Prescribing Day: Strengthening Communities Together
Today, March 26th 2026, we celebrate National Social Prescribing Day in Pembrokeshire — a moment to recognise the power of connection, community, and compassionate support in improving people’s health and wellbeing. For the Strengthening Communities team at PAVS, Social Prescribing Day is more than a date in the calendar. It’s a celebration of people — the individuals, volunteers, community groups, and professionals who work together every day to make a meaningful difference in our County. Social prescribing is about looking beyond traditional services and recognising that health and wellbeing are shaped by much more than clinical care. It connects people to activities, groups, and support within their communities — whether that’s joining a local club, accessing practical help, or simply finding a place to belong.
In Pembrokeshire, the social prescribing service is delivered by our team of Community Connectors, working in partnership with health board and local authority colleagues, GP surgeries, and the voluntary sector. This collaborative approach ensures that people of all ages receive joined-up, person-centred support that truly reflects their needs. The service is further strengthened by its close connection with the Pembrokeshire Community Hub — a one-stop shop for community information and support. Together, these services work seamlessly to help individuals access the right guidance, opportunities, and local resources, making it easier for people to take positive steps towards improved wellbeing. Across our communities, we see first-hand how powerful these connections can be. A friendly conversation can reduce loneliness. A new activity can build confidence. A supportive network can help someone navigate difficult times. These small steps often lead to life-changing outcomes.
Recently, Strengthening Communities Team Programme Manager Sophie Buckley and Community Connector Team Leader Kathryn Gibson represented PAVS at the Welsh Government National Framework for Social Prescribing showcase in Cardiff. The event marked the launch of a core set of standards and values for social prescribing, helping to shape a consistent, high-quality approach across Wales. This national work reflects the importance of collaboration, shared learning, and a strong foundation for the future of social prescribing.
