What do we do?

Eight Community Connectors working in six integrated areas across Pembrokeshire.

The Community Connector service is a key element of the Strengthening Communities Programme, a partnership approach with Pembrokeshire County Council, Hywel Dda Health Board & third sector. 

It is a community and citizen-centred approach to improving health and wellbeing in Pembrokeshire, beginning with a What Matters conversation and often involving social prescribing linked to the 5 ways to wellbeing. 

How can I access the Community Connector Project? 

If you need support, you can make a referral to the Community Connectors by contacting the Pembrokeshire Community Hub: 01437 723660 (between 9-5pm weekdays only).

Community Connector for Children, Young People & Families

Sarah Harvey at PAVS works with families and individuals to provide signposting, advice, support and information. She works with groups, organisations, projects and agencies to try and ensure that anything affecting families, children and young people is supported in Pembrokeshire. She also works with professionals and practitioners to promote their services and to connect them to each other. Sarah is part of the Pembrokeshire Families Support Network.

Supportive Communities – Age & Dementia Friendly

Angela Reid at PAVS works across Pembrokeshire with local people, communities and organisations to develop and support the develop a network of Dementia Supportive and Age Friendly Communities and activities, which provide local support enabling people living with dementia, older people, their carers & families to have the best quality of life possible.

Office Hours

Our staff are now working in a hybrid way which means they may not be in the office, but working from other venues or home working. Due to changes in the way we now work, we are not holding meetings or running training events in person from our premises. However, if you wish to meet with a member of staff then please email them directly or call their business mobile number. The staff contact list is kept up to date with the most recent information.

If you do need to call at our premises, we are open from 9.00am to 5.00pm Monday to Thursday and 9.00am to 4.30pm on a Friday (except on Bank Holidays). Between 9am and 10am, and after 2pm, Monday to Thursday and all day Friday you may need to ring the bell (situated on the right hand side of the door). If you are just delivering mail, then please use the letter box on the front door.
